Notice is hereby given that Knoxville Infectious Disease Consultants, Inc. has been dissolved and is in the process of winding up and liquidating its business and affairs. All persons with claims against the Corporation must send them to the Corporation in writing at 2210 Sutherland Avenue, Suite 110, Knoxville, Tennessee 37919.
The following information must be included with each claim:
(a) The name and address of the claimant,
(b) The description and basis of the claim,
