Pursuant to the provisions of Tennessee Code Annotated § 48-64-108, SERENE MANOR HOSPITAL, INC., (the “Company”) hereby gives notice of its dissolution.  The Company hereby requests that any persons with claims against it present them to the Company in accordance with the following.

The following information is required to be included in each claim:(a) the name, address, and telephone number of the claimant; (b) the nature of the claim, including any substantiating documentation of the claim; (c) the amount of the claim; and (d) the date the claim arose.
