Heart Centers of America, Inc. (“HCA”) filed Articles of Dissolution with the Government of the District of Columbia effective May 29, 2020.  Any person or entity claiming that HCA is indebted to it may file a claim as follows:

(a)  Mail the claim to P.O. Box 5478 Knoxville, Tennessee 37928;

(b) Include the total amount of the claim together with all appropriate supporting documentation; failure to provide supporting documentation may result in rejection of the claim;

(c) Claimants who have not received written notice or claimants who have previously filed a claim that HCA did not act upon or claimants whose claim is contingent or based upon an event occurring after May 29, 2020 have up to three (3) years from the date of publication of this notice in which to enforce such claim pursuant to District of Columbia Code §29-412.07.
